Two forms of identification (driver’s license, state ID, tribal ID, passport, government ID), current health insurance card, a list of current medications and supplements with dosage information, a list of other physicians you see and their contact information, current immunization record, discharge paperwork from any recent hospital visit, and a list of questions.

While traditional insurance plans do cover some treatment options, coverage will depend on the treatment plan determined by you and your provider. When you call to schedule your appointment, a patient services representative will verify your insurance and confirm that it is accepted. You will be asked to provide a current insurance card on the day of your appointment. Even if your insurance plan does not cover naturopathic medicine, you can use your HSA account to pay for your care. For more information, please call 480-970-0000.

HSA and FSA funds can be used towards your appointment with your naturopathic physician as a qualifying medical expense, meaning you may be required to have a recommendation from your general practitioner. Contact your insurance provider for more information.

If this is your first visit, please complete your new patient forms on your Patient Portal prior to your arrival on campus. By completing your patient forms ahead of time, you allow your provider more time during the appointment to uncover the root cause of your health concerns. If you have not set up your Patient Portal, please call 480-970-0000.

Sometimes it’s unavoidable. If you know you’re running late, give us a call. We’ll do everything we can to work you in, however, we may need to reschedule your appointment for another day.

To allow our physicians to accommodate as many patients as possible, we require a 24-hour notice for any cancellations or reschedules. If less than 24-hour notice is given, or you do not show for your appointment, you will be charged $25.00, which must be paid prior to your next visit. For a Supervised Shift, new patients can expect a rate of $96, while established patients can expect a rate of $64. Private Provider visits are offered at a higher rate, please call 480-970-0000 for Private Provider visit rate information.

Supervised student shift appointments are usually longer in time (1 to 1 ½ hours) due to the detailed initial intake and discussion with the supervising physician overseeing their clinical shift regarding the best steps for your care. This licensed physician will make all final decisions on your health care.
